Johnson Creek Secure Residential Treatment Facility is scheduled to be open mid-November, 2009.  This 8-bed facility is a beautiful, newly constructed, craftsman style home that is tucked away in the quaint community of Milwaukie, Oregon.  Johnson Creek will promote community safety and the psychiatric recovery of individuals who are under the jurisdiction of the Psychiatric Security Review Board.  Residents living at Johnson Creek will be ready for community integration with a high level of support and supervision. CODA - 8041 Recovery House            

  • To provide intensive residential treatment and rehabilitation for individuals with mental health disorders transitioning from secure treatment settings back into the community.
  • Clients whose care is managed within the Extended Care  Management Unit 
  • Individuals who accept structured residential treatment as a step in their recovery from behavioral health conditions 
  • Individuals with co-occurring disorders who will benefit from the continuum of substance dependence and mental health services available at CODA

Treatment Philosophy:
CODA’s mission is to significantly reduce mental health and alcohol and drug problems and their social, health and behavioral consequences through the use of evidence-based practices, partnerships with other social and health care providers, and advocacy for effective public policy.
This mission is driven by a vision that foresees a community in which mental health and alcohol and drug problems are recognized as public health issues that are both preventable and treatable. We envision a society in which people with a history of mental health and alcohol and drug problems, people in recovery, and people at risk for these problems are valued and treated with dignity and respect; where stigma, prejudice and discrimination and other barriers to prevention and treatment are eliminated.
CODA’s integrated clinical model has adopted the Dartmouth Psychiatric Research Center’s four stages of treatment. This work is particularly relevant to individuals with co-occurring disorders. The four stages (Engagement, Persuasion, Active Treatment, and Relapse Prevention) guide treatment planning processes. Initial and ongoing assessment of the client’s level of motivation is paramount to guiding appropriate interventions. Most programs claim to use these concepts. Few, if any organize whole systems of care around them.

Cedar Bay is a 5 bed Residential Treatment Home located in Coos County in the city of North Bend, Oregon.  Cedar Bay serves 5 adults under the jurisdiction of the PSRB who are discharged from the Oregon State Hospital.  Residential care and mental health treatment services are provided to enable the residents to successfully transition back into the community.  Cedar Bay is in an ideal location, with fishing and other recreational activities being literally twenty (20) feet out the back door, and yet the home is still only a few blocks from downtown North Bend and county services. Supported Housing

Bay Apartments is a 10-unit transitional supportive housing program providing studio apartments to adults with a mental illness living in Coos County for up to 18 months. Morrow Bunkhouse is a 2-bedroom home that provides supportive housing for 2 tenants and is located on Columbia River Ranch property in Boardman, Oregon.  Mental Health services, as needed, are provided by Morrow County Behavioral Health.  Morrow Bunkhouse features 2 bedrooms, a kitchen, living room, laundry room, patio and yard.  Residents live semi-independently providing for most of their own needs such as grocery shopping and preparing most of their own meals.  Residents commonly work or attend school.  The staff at Columbia River Ranch is nearby for assistance. Luke-Dorf’s PLUSS Apartments are Semi-Independent Supported Living Facilities located in Tigard, OR.  With spacious, private bedrooms and attached bathrooms combine with a shared kitchen and living area, they offer the privacy of an apartment but the support of roommates close by.  The apartments are within walking distance of multiple parks, green spaces and public transportation lines as well as shopping and employment opportunities along Hall Blvd.  In addition, the apartments are next door to the Sage House where they have access to Dual Recovery and Art Therapy Groups.  Residents must meet Affordable Housing income guidelines to qualify, and agree to house rules regarding clean and sober housing.  Program Manager: Elizabeth Adami, 503-726-3706, eadami@luke-dorf.org

 

 

Sage House is a five-bed supported housing facility located less than 1 mile from Client Services West in Tigard, Oregon. The Sage House provides a clean and sober living environment for adults with substance abuse disorders and persistent mental illness. It is a modified therapeutic community based on the understanding that people with Dual Disorders can successfully achieve lasting and fulfilling recovery. The Sage House program is based on self-help, where the residents help each other and are actively responsible for their treatment. Residents have access to an on-site group room, where Dual Diagnosis Anonymous meetings, Art Therapy, and Dual Disorders groups are held. Crisis Respite Resources

Coos Crisis Resolution Center is a 6 bed Residential Treatment Facility in Coos Bay, Oregon providing an alternative to emergency, short term psychiatric hospitalization for those patients whose illness allows them to be treated in a residential setting.  The CCRC is conveniently located in a nice residential neighborhood, close to services, directly across the street from the Bay clinic, and just down the road from Bay Area Hospital Care is provided by a 24 hour, 7 day a week trained and caring staff with supervision by a Psychiatric Nurse Practitioner and a Mental Health Professional.  Contact: Katherine Parker, Administrator Email: kparker@columbiacare.org Phone: 541-266-8480 Fax: 541-266-8479

 

 

ShelterCare Royal Avenue Crisis Respite ProgramEugene, OR Royal Avenue Program provides crisis respite for individuals referred primarily by Lane County mental health system partners.  The twelve bed facility is transitioning to twenty-four beds for crisis respite in January 2010.  Average length of stay is seven to ten days during which time program participants are supported to resolve the immediate mental health crisis and connect to community resources for ongoing support as they return to the community.   The facility has recently completed construction of a spacious community room for program participants. Pony Creek is a beautifully renovated home located in North Bend, Oregon and developed for the purpose of providing short-term planned and crisis respite treatment foster care for children residing in Coos, Curry and coastal Douglas County. As resources were previously located as far away as Portland, Pony Creek is a great asset as it allows families to receive care in their own communities. Families are now able to be involved with the child throughout the stabilization process, enabling them to be an active part of the solution, reducing family disruption, and improving outcomes.
